Anti-inflammatory fast food options: what to order at 9 chains
Fast food is not automatically off the table on an anti-inflammatory diet. The menu is. Most chains sell at least one order that is mostly beans, vegetables, grilled protein and a decent fat; the trick is that it is never the combo on the poster.
Below are five ordering rules, the order and the skip list at nine common US chains, and the swap table that explains why. Menus change and we do not list calories; the logic holds whatever the current menu is.

Five rules that work at any counter
Memorize these and you do not need the chain list below.
- 1
Bowl or salad over bun or tortilla
Removes the refined flour, which is often the single worst-scoring item on the tray.
- 2
Grilled over fried
Frying adds seed oil to everything it touches. Fries score 8; the same potato baked scores far lower.
- 3
Beans, vegetables and avocado as the bulk
Fiber and fat that work in your favor. Avocado scores 2.
- 4
Sauce on the side
Creamy sauces and dressings are mostly seed oil and sugar. You control the amount.
- 5
Water, unsweetened tea or coffee
Soda scores 9. The drink is the easiest win on the whole order.
Chain by chain: order this, skip that
Ranked roughly from easiest to hardest. Bowl chains are built for this; burger chains are not.
| Chain | Order | Skip |
|---|---|---|
| Chipotle | Burrito bowl or salad: brown rice, black or pinto beans, fajita veggies, chicken or sofritas, fresh tomato salsa, guacamole | The tortilla, chips, queso, sour cream |
| Sweetgreen | Any warm bowl or salad with a whole grain, greens and a protein; ask for dressing on the side | Extra dressing, crispy toppings |
| CAVA | Greens and grains bowl with lentils or brown rice, hummus, vegetables and grilled chicken or falafel | Pita and pita chips |
| Chick-fil-A | Grilled nuggets, Market Salad, Kale Crunch side | Breaded sandwich, waffle fries, sweet tea, milkshakes |
| Panda Express | Super Greens as the base, with String Bean Chicken Breast or Grilled Teriyaki Chicken | Fried rice, chow mein, orange chicken |
| Subway | A salad or a wrap loaded with vegetables, grilled chicken or tuna, oil and vinegar | White bread, processed cold cuts, creamy sauces, chips |
| Wendy's | Plain baked potato and a small chili | Fries, bacon toppings, Frosty |
| Starbucks | Rolled and steel-cut oatmeal with nuts and fruit, egg bites, unsweetened tea or black coffee | Pastries, sweetened frappuccinos |
| McDonald's | Honestly little: apple slices, black coffee. If you must, a plain hamburger without fries | Fries, soda, breakfast sandwiches with sausage |
The pattern across all nine: the good order exists, it just requires you to drop the bread and the fryer. At a bowl chain that costs nothing. At a burger chain it leaves very little.
Why: the tray, item by item
Scores on our 1 to 10 inflammation scale (1 strongly anti-inflammatory, 10 highly inflammatory).
| Usual item | Score | Swap | Score |
|---|---|---|---|
| Fries | 8/10 | Baked sweet potato | 2/10 |
| Soda | 9/10 | Unsweetened green tea | 1/10 |
| Bun or white tortilla | 7/10 | Brown rice bowl base | 4/10 |
| Bacon | 8/10 | Grilled chicken | 4/10 |
| Mayo-based sauce | 7/10 | Guacamole or avocado | 2/10 |
| Cheese or queso | 6/10 | Hummus | 3/10 |
| Milkshake or ice cream | 8/10 | Fresh fruit cup | 2/10 |
Two of these do most of the damage: the fries and the soda. Drop those two and a mediocre order becomes a reasonable one, even if you keep the bun. For the head-to-head, see fries vs sweet potato and soda vs green tea.
Build-your-own counters: a cheat sheet
Bowl, poke, salad and Mediterranean counters all work the same way: you walk down a line and pick one thing per station. This is the pick at each station, whatever the chain.
| Station | Pick | Pass |
|---|---|---|
| Base | Greens, or half greens and half brown rice, quinoa or lentils | White rice, noodles, tortilla, pita |
| Protein | Salmon, tuna, grilled chicken, tofu, falafel if baked, beans | Fried or breaded anything, chorizo, bacon |
| Vegetables | As many as they will give you: peppers, cucumber, onion, tomato, cabbage | Nothing to pass here |
| Extras | Avocado or guacamole, hummus, seeds, a few nuts | Cheese, queso, sour cream, crispy onions, croutons |
| Sauce | Salsa, lemon, olive oil and vinegar, tahini on the side | Creamy or sweet sauces: ranch, spicy mayo, teriyaki glaze in quantity |
The extras and the sauce are where a good bowl quietly turns into a bad one, because they are added a spoon at a time and nobody counts spoons. Pick one creamy element at most, and ask for the sauce on the side so you decide how much goes on. A salmon poke bowl over greens with avocado and a light sauce is about as good as takeout gets: salmon and avocado score 1 and 2 on our scale.
Three situations where it gets hard
Breakfast drive-through
Most breakfast menus are bread, processed meat and sugar. Oatmeal or egg bites at a coffee chain beat any breakfast sandwich. Black coffee is fine: it scores 3.
Road trip, only burgers
Plain burger, no fries, water. Eat the vegetables you packed. One bad meal on a travel day does not undo a week.
Airport food court
Look for the bowl, salad or Mediterranean counter first. Grab nuts or fruit from a news stand instead of a pastry.
If you can plan ahead, packing lunch beats every option here. Our anti-inflammatory snacks list covers what travels well.
Anti-inflammatory fast food: common questions
What fast food is anti-inflammatory?
Bowls and salads built on beans, vegetables, whole grains, grilled protein and avocado. A Chipotle burrito bowl with brown rice, beans, fajita veggies, chicken, salsa and guacamole is the easiest example. Chains built around bowls (Chipotle, Sweetgreen, CAVA) make it simplest; burger chains make it hardest.
Is Chipotle anti-inflammatory?
It can be one of the best fast food options. Skip the tortilla, chips, queso and sour cream, and build the bowl from brown rice, beans, fajita vegetables, chicken or sofritas, salsa and guacamole.
What is the least inflammatory thing at McDonald's?
There is not much. Apple slices and black coffee are the safe picks. A plain hamburger without fries and soda is the least bad meal, but the bun and processed patty still score poorly.
Are fast food salads actually healthy?
The greens are. The dressing, cheese and crispy toppings can undo it. Ask for dressing on the side, choose oil and vinegar where possible, and skip anything fried on top.
What drinks should I order at fast food places?
Water, sparkling water, unsweetened iced tea or black coffee. Soda scores 9 out of 10 on our scale, and sweet tea and milkshakes are not far behind because of the sugar.
How often can I eat fast food on an anti-inflammatory diet?
The rules above make most visits workable. What matters is the overall pattern: if fast food is a few meals a week and you order bowls, the rest of the diet does the heavy lifting.
